CAMPER ACCOUNT INFORMATION REQUIRED
To expedite registration, please update each individual child's account accordingly:
Please review and update each child's account information for accuracy by logging into your account, click on add/edit account and then click on the child's name. You will need to do this for each child even if they have the same emergency contacts.
FOR PLAYGROUND WE REQUIRE:
*List/Update the primary parent or guardian
*List/Update TWO Emergency Contacts & phone numbers on each camper's account.
*List any allergies/medical conditions for your camper
*Under Allergies/medical conditions, list if camper may need to self-administer any medications. (A separate form will be required for you & your child's physician to complete).
In an emergency, every attempt will be made to contact the parent or guardian listed on the account. If we are unable to reach the parent or guardian listed, we will contact the emergency contacts listed in the order entered.
Field Trip & Additional Waivers
Must pay by Credit Card or debit card when registering from home.
In-office registrations will still require online registration by a parent or guardian. An Ipad will be available for parent or guardian use. Camp may then be paid by credit, debit, or check. Please be sure to leave time for completing on-line registration in the office this process may take up to 30 minutes. Our office hours are: M-Th 8:00 am - 4:30 pm or Fridays 8:00 am - 12 pm.
All field trips must be paid on line or at the Community Center. Staff are not allowed to accept payments at Bungay. NO REFUNDS will be given once trip is paid.
Depending upon the week your child attends, additional waivers may be required. PLEASE SEE THE FORMS SECTION BELOW FOR THE additional WAIVER FORMS. ALL WAIVER FORMS SHOULD BE Dropped off at Camp or emailed to email@example.com. NO LATER THAN THE REGISTRATION DUE DATE.
